Team Eligibility

The following are forms that need to be filled out to be eligible (NOTE: only the first one needs to be filled out before tryouts, the rest can be done after the team is selected). Athletes cannot play in any game unless all the following forms are filled out and fees are paid:

• To tryout for the Herriman High basketball teams in November, THIS form must be filled out.

• The fees need to paid in the main office ($75 participation, $75 uniform rental)
